We are hiring a full-time CNA to join our Elmwood Care Center Skilled Nursing. Performs the duties of a nursing assistant as assigned by the Charge Nurses, Director of Staff Development and/or Director of Nursing. Is accountable for all patient care given during shift. Responsible to adhere to all nursing care standards.


  ESSENTIAL DUTIES AND RESPONSIBILITIES: Oral hygiene: o A.M. Care (bath, shower, bed, etc.) o P.M. Care (bath, shower, bed)
  • Provide clean linen when appropriate:
o Make unoccupied bed o Make occupied bed o Change and straighten linen to remove wrinkles
  • Give and remove bedpan/urinal. Clean and store in the same designated place after each use
  • Assist community members to and from bathroom.
Nursing Assistant will be responsible to assist with such duties in the following areas:
  • Documentation
o Perform ADL charting o Bowel & bladder training and documentation o Meal percentages charting
  • Positioning residents:
o Position community members according their clinical and physical condition o Change community members’ position appropriately and as frequently as needed o Observe proper body mechanics
  • Dietary needs:
o Ensure community members receive prescribed diet o Place tray conveniently for community members o Assist or feed community members as needed o Encourage good food intake to promote healing o Provide all snacks and nourishments at ordered time (especially for diabetic patients, etc.) o Remove tray and inform Charge Nurse of community members’ appetite (good, fair, poor, or refused) o Provide fluids as needed
  • Vital signs: The Nursing Assistant will take vital signs as instructed by Charge Nurses.
o Temperature as ordered o Heart rate (pulse) o Respiration rate – including description (rapid, shallow, deep, regular, or moist, etc.) o Blood pressure o Assess pain scale o Notify RN or LVN of any change or abnormal findings o Record vital signs on designated form at times requested by Charge Nurse
  • Transfers:
o Assist resident to wheelchair or stretcher o Accompany resident to a new room or vehicle when necessary o Provide with call light and orient to new surroundings o Report the resident’s condition at the end of shift to Nursing Assistant resuming care o Pack and take inventory of belongings for any routine transfers
  • Safety:
o Ensure beds are in the lowest position and locked at all times o Ensure that community members are secured appropriately when transporting by wheelchair or stretcher o Assist community members in moving, sitting or ambulating as necessary o Verify that needed articles are within the community members’ reach (water, tissues, bedpan, etc.) and rooms remain clutter free o Check that side rails are in position as indicated o Ensure floors are always dry-wipe up spills immediately o Ensure good lighting in halls and bathrooms
  • Admissions:
o Bring community members to and from the room to other areas of the facility o Assist community members to bed and check for armband and nameplate on the door o Obtain vital signs, height and weight, noting allergies and reporting to RN or LVN o Assure the community member is comfortable and oriented to surrounding areas o Assist RN or LVN with initial patient interview o Perform CNA admission checklist o Issue bath basin, bedpan/urinal and fresh water
  • Discharge:
o Help community members collect personal belongings o Assist community members to their vehicle on a wheelchair o Remove linen from the unit o Return equipment from the unit back to the storage room JOB SPECIFICATIONS: Full-time: 3:00-11:00pm & 11:00-7:00pm   Due to California State regulations, we are required to have all our applicants pass a Background Check, Physical, and TB test. *FOR YOU TO BE CONSIDERED PLEASE SUBMIT YOUR RESUME AS A PDF ONLY* EXPERIENCE:
  • Must have active CNA license.
  • Preferably someone who has worked with seniors
  • Requires some knowledge of the nursing services field. Must have Nursing Aide certification.
  • Must be state licensed at time of hire.
  • CPR and first aid certified preferred.
